Online Membership Application

Welcome

This is the SMC Community & Employees Multipurpose Cooperative Online Membership Application System.

Before you proceed, be ready to upload the following:

  • Copy of a valid government-issued ID showing your full name and date of birth — UMID, SSS, GSIS, PhilHealth, Pag-IBIG, Driver's License, Passport, or similar — with three specimens of your signature.
  • Proof of address dated within the last three months, such as a utility bill (water, electricity, telephone), credit card statement, or bank statement.
  • Recent 1x1 or 2x2 ID photo, if available.

Please make sure the mobile number and email address you provide are the ones you actually use. These will be SMC's means of communication with you regarding your application and, once accepted, future notices on your membership.

Further, part of this online system is a short Pre-Membership Seminar video which you are required to watch before you can complete the submission of your application.

Finally, please be prepared to pledge the following:

  1. Minimum Share Capital Subscription of 400 shares (Php 40,000.00).
  2. Initial Savings Deposit of Php 1,000.00.
  3. Membership Fee of Php 100.00.
  4. Passbook Fee of Php 50.00.
It is also understood that my accumulated share capital shall not be withdrawn during my membership with the cooperative. The Treasurer of the Cooperative is also hereby authorized to collect from the Paymaster/Cashier out of my salary the sum indicated above.
